1. Encoder fault
2. Hiperface encoder fault
3. Encoder wiring incorrect
4. Internal drive cause

Corrective action

1. Replace the encoder
2. Check the encoder wiring and ensure that it is correct
3. For a Hiperface encoder, recalibrate the encoder sine/cosine AD coefficients
4. Restart the drive
5. Replace the drive
