Cause

1. Drive U, V, W outputs have broken wires, poor connections, or similar problems
2. Motor impedance too high
3. Drive internal current sampling circuit abnormal

Corrective action

1. Check the motor U, V, W wiring and ensure secure connections
2. Replace the motor
3. Replace the drive
4. If not required, disable drive output phase loss detection using parameter 0x20A1.Byte0
